When you have decided what trainings you would like to apply for, you can download the training form NSCTNG 001 for all trainings except Coast Guard. For Coast Guard or other locally arranged training, download form NSCTNG 003. Fill out one form for each training (the forms can be filled out on the computer). Forms will have to be signed by the CO or Training Officer. You will have to purchase a certified check or money order to mail with each training application. COTC's will not accept a personal check.

Use to authorize NSCC/NLCC Cadets for up to five 5 days of locally arranged training or ten 10 days of training with the US Coast Guard.
